exam access arrangements at uni without MH diagnosis

i have anxiety which really affects my ability to take exams so for gcses and a-levels i've had rest breaks and small room to help me get through exams. i'm trying to apply for AEA (alternative exam arrangements) at bristol (starting in september) as well as receiving some kind of mental health support while there, but i don't know if i have a current diagnosis of anxiety - will this impact my ability to access these exam arrangements/support?

the reason i'm unsure about my diagnosis is that i have been previously diagnosed with 'mixed anxiety and depressive disorder' by camhs/nhs but when i went to look at my gp records on the nhs app it said my diagnosis is 'past' and ended back in february. i didn't know my diagnosis had changed and honestly don't think that it's reflective of my day to day life to say i don't experience anxiety. i've been on antidepressants for around 6 months to help with my symptoms of depression, but my anxiety has remained quite significant.

i tried contacting my gp about it but they said that the diagnosis will always be on my records - what does that mean? is it that i will always be diagnosed with mixed anxiety + depression or that it will always be on my gp records as something i have experienced? i figure it means the second one, but that's not what i was asking and they're closed all weekend so i can't ask for clarification

sorry for such a long ramble about it but it's really stressing me out and i have exams starting next week!!!!!
