What else should you tell the teacher writing your University Reference (UK)?

We’ve been asked to fill in some details to provide to our teacher which they will use to write our reference.

Is there any key information I should provide to improve my reference or things which will look good.

Also are there any additional things I should mention which people maybe forget to tell?

Evidence of interest in this subject outside formal studies - ie. any project / competition, event etc.
The UCAS reference covers 3 sections (and is the same length in total as the PS).

Section 1: Provide an overview of your relationship with the applicant (mandatory)
Section 2: If applicable, enter any information about extenuating circumstances which may have impacted the applicant's education and achievement
Section 3: Outline other supportive information specific to the applicant and relevant to the course(s) applied for that you think universities and colleges should be aware of
