How to rebuild old friendships?

Hey, I'm 17m and really want to reconnect with an old friend. We used to be good mates way back in year 7 and 8, but I don't think I've spoken to her for maybe 3 years at this point (not because of any kind of falling out, we just drifted apart). We've both changed a lot since then, but I'd really like to rebuild that connection. Some added context that might help is that she's trans and transitioned about a year or so after we stopped talking, and our social circles are so disconnected that I don't even know her name or know anyone who does, which I feel really bad about.

The name thing I'm not sure about. Maybe try and stalk Instagrams or smth.

I had a friend who I was rlly close with in yr 10, we had a bit of a falling out in yr 11 and then she moved 6th forms and I didn't even know she was leaving. I found it incredibly weird how we went from being inseparable to being nothing at all. So I just reached out and straight up told her that, that we had such a good friendship I didn't think was worth losing and I want to stay friends. Then we had a huge catch up and talk frequently.

So I guess just be honest, 'Hey, how have you been etc etc I know we've drifted but I'd really like to rekindle (/keep talking or idk a more modern way to say it lol) the friendship'
