Whatever is more comfortable for you. Try contacts. If you have a really bad eyesight contacts will give you a better field of view and most likely a better visual acuity and one of the most important is a more natural vision (things will appear more natural sizes and distances).
Nothing is wrong with glasses, but with a high prescription they are not always the best solution. Glasses are the easiest and usually the cheapest option, you can pick up a better looking frame, one that suits you better.
As for surgery... it is risky and expensive. I'm personally very afraid of it. I've seen so many complications from this surgery. I wish I could be suitable for it 100% but every small issue ( frequent infections/allergies, even minor change in the prescription, focusing issue, eye fatigue etc...) can make an impact during the surgery and recovery and cause complications. Most of them are not really treatable.
Dentistry requires good eyesight for up close and intermediate distances. You should fit your glasses/contacts for those distances.
