Access arrangements and A levels

Hi I was wondering how to go about asking for access arrangements for my A levels, I’m in year 12 currently. I recently emailed the person in charge of it saying that I have severe pms and tinnitus, however when I spoke to them they only talked about exam support for tinnitus and didn’t mention the other issue (which has a greater impact on me). When I spoke to him irl I didn’t mention my severe pms too as there was other people in the room so I was too uncomfortable to talk about it.

They offered me to wear head phones with white noise to help with my tinnitus however they didn’t talk about my severe pms, I don’t think they fully read the email. To put things into context last year a few days before my GCSEs my pmdd got so bad that I ended up staying in hospital overnight due to something I did bcs I was so depressed.

I was wondering if I should ask again for access arrangements for pmdd and explain how it affected me in my GCSEs or should I just leave it. I’m not sure if I can get access arrangements for this as last year in GCSEs I didn’t get any support for it, pls can anyone let me know what support is available for this (I’m currently on antidepressants which helps my severe pms however it doesn’t fully eradicate the issue if ykwim)
