Pn1 form questions

I’m completing a paper pn1 form for sfe and wanted to know-

Do I send my section of the form with the sections my parents completed in the same envelope?

Do I send my photocopied birth certificate in the same envelope as the pn1?

Yes, you can send all of these things together if you are sending the PN1 by post.

On section 4 page 21 it says ucas campus code but the AUB uni I’m applying to doesn’t have one as everything’s taught at one site- do I leave that box blank?
