advice for nursing booklet

hello all, I'm in my first year studying mental health nursing. I've finished both both placements and completed my hours, my assessor and supervisor have signed everything.
However, there's a part in my booklet the episode of care, and standards of proficiency, which my assessor needed to signed (yes I've achieved it and a small comment), unfortunately on the day she was busy but managed to say I've achieved all the standard proficiencies but was unable to write a small comment beside it. she signed it in the end.
i don't know if my academic assessor will tell me to go back to my placement and ask for my assessor to write comments.

I don't know what to do, as tomorrow is my last day and my academic supervisor will be looking over my PAD I won't have time afterwards as ill be heading home, and I live 3 hours away from my uni and won't have time.

You are solely responsible to make sure that you have everything signed off by your practice mentor on placements.
I would suggest that you just discuss it with your academic assessor and see what they tell you okay.
Your academic assessor might ask you to go back to the placement this happened at to make sure you get enough signed up to date.

I had a student I was mentor to with similar problems as yourself but I handed her over to another nurse because I was going on holiday as the student still had a couple of weeks left to complete the placement but unfortunately the dedicated nurse never completed the student coursework information and eventually her placement officer at university contacted me asking about it, we agreed to ask her to come and get it all signed off.

Hello, thank you for the reply.
My practice assessor has signed everything off, there’s a section in my PAD which is about episode of care, she’s signed it off and wrote one comment and said yes to everything (yes-achieved)
I don’t know if the comments are necessary because she has signed off everything

When I sign off any students midwives I don't necessarily put down any comments in the box. - if I do put anything in the box it's usually things like - ( for example - brilliant at almost everything they been asked to do but maybe could do better with their communication skills or something like that or another one is couldn't fault them throughout the placement as very excellent student and will make a brilliant midwife when qualify. ).

It's generally up to the individual supervisor/ mentor whether they want to put any additional information on the students PAD okay. I have not been told that it is compulsory to write anything in the comment box etc.

I don't think any of the students I taught over the last couple years has had any issues with no comments in there PAD box or even on the students coursework folder.

You' could ask your assessor prior to handing over your coursework if it's necessary for you to have your mentor/ supervisor to write anything in the comment box.
