Not meeting conditionals but getting good alevels?

I'm in year 12 and i'm anxious over my upcoming predicted grades.
I was wondering how likely or what the possibility is to get into uni first year, through weak predicteds, however strong a level grades.
If my predicted grades are unable to get me any offers would the schools be willing to offer me a place on results day if I am able to gain really strong A-levels: a *hyperbolic* example would be getting predicted BBB and no conditional offers but then getting A*A*A* in your actual results day.
In my context I'm applying for medicine.

No generally you would have to take a gap year and reapply with you already achieved a level grades. There may be a few places in clearing for medicine but it’s very rare
I think there's a couple of med options which don't use predicted grades. So yes, possible, but would limit your choices.
