If a place as your firm is confirmed on Thursday, then you will have the option to decline it and drop into Clearing. You don't have the option to opt for your Insurance choice instead (although you may be able to go there via Clearing).
You don't have you decide in results day, but obviously the longer you leave it, the less chance there is that you'll get something suitable via Clearing. In theory, you could wait as long as enrollment day at your firm - but that wouldn't be terribly polite.
You can request that your firm defer your confirmed place, but they have no obligation to do so. If they do defer it, you can always decline it later.

Understood. The fact that you are "not sure I still want to go to my firm choice" is a little worrying, this late in the process. Are you hoping that a really attractive alternative becomes available via Clearing, and that will help you make-up your mind?

It’s more to do with the course than the Uni, I had applied to quite a specific course and I’m not sure now - it’s longer than other courses and the extra expense is an issue q

Yes it is the cost, it would be higher because the course is longer and there is less help with this

I am thinking of changing to a Uni with lower fees and cheaper accomodation now or take a gap year to work first

Have your requested your scripts? This is free and if you can get your teachers to look through them first, they should be able to advise you if a review of marking is worth it.

I was thinking of doing this but my schools told me its for AQA only, and I do OCR for all subjects, so if I request the scripts they can't be reviewed afterwards, or I can request scripts after they've been reviewed, so I think I have to go in a bit blind

I think your school has misadvised you. They are available according to the OCR website and the pdf:
