UCAS / Uni application Extenuating Circumstance?

I am parent to a daughter who will be completing her UCAS application in the coming weeks

There are some Extenuating Circumstances we need to include because they have definitely impacted grades so far and will still be an issue this coming year - but I don't know the process for stuff like this

Q1 - is there somewhere in UCAS you put this, or is it each Uni that you tell via a different channel?

Q2 - its a health thing, so do we get a GP letter as evidence? Do GPs have a proforma for this or do we need an appointment to agree wording / content
extenuating circumstances should be included as part of the reference.

On top of this some universities will have a form or process to follow after an application is submitted to notify or provide evidence to be taken into consideration. This would be explained in the university’s admissions policy
