Medicine offers

This might be a silly question but I keep seeing people get grades lower than 3 A's for eg aab or abb and they're never able to get into med, but don't contextual offers exist and for some of them the criteria of being eligible is quite low. Can someone pls explain

There are about 30k applicants for Medicine each year for a grand total of ~9k Med school places. Even with the existence of contextual offers, not everybody can get a place and then not everybody that gets AAB is eligible for contextual offers.
