Yes and yes, but you can (and should) use the same account you use last year. You can reuse your PS but it would be nice to touch it up for what you've been doing in the last year to further prepare yourself for university study.

I logged into the same account as before but now the UCAS personal id is changed and there is no history of the account from before somehow. it's just asking me to submit a new application. And also I cba adding a reference in the ucas form because I doubt my teachers would answer, so will I be cooked if I submit my application and then apply to clearing without a reference or is it not that deep. ALSO thanks for the quick reply

The personal ID issue will sort itself out when you apply.

Ideally you should have a reference, especially if you want a competitive clearing choice (e.g. a usually competitive course from a RG or 'top' university) but it's up to you and it won't be the be all and end all for all unis.

Yes that's correct, you'll need a new application for this year.
You should be fine to submit without a reference, most unis are concerned about grades only tomorrow. However it could be that some have a problem with it, if so then either pick a different uni or come back here and we can try to help  Also worth contacting your teacher if you end up in that situation

When you apply they will ask for your UCAS ID.

If you’ve submitted an application they can access your entire application using your UCAS ID.

If they make you an offer then you add them on ucas as a clearing choice and UCAS sends your full application to them.
They then check that everything in your application is in order and as described over the phone and ask you to send through anything missing
