Malcolm is describing the state of Scotland under Macbeth's tyrannical rule. Malcolm personifies Scotland as a suffering woman to show how the country is constantly wounded by the ongoing oppression and violence created by Macbeth. Furthermore, his use of semantic field related to pain, "weeps", "bleeds", "gash", evokes a sense of sorrow and pity, further highlighting the intense suffering that Scotland endures, suggesting that the nation is being continuously harmed, with no relief in sight - each new day brings further damage, symbolizing the unrelenting nature of Macbeth’s tyranny. Perhaps, at this point, Shakespeare could be highlighting the drastic consequences of a tyrannical rule.

At this point in the play, Macbeth is deeply troubled by guilt and paranoia after having murdered King Duncan to seize the throne. The metaphor "full of scorpions" vividly expresses the mental anguish and the tormenting thoughts that plague him - he feels that his mind is infested with dangerous, stinging creatures, symbolizing the relentless and painful nature of his conscience, which is further emphasised by his exclamatory speech. Here, Shakespeare masterfully captures Macbeth's descent into madness, driven by his guilt and fear of losing power, hence warning the audience of the destructive consequences of unchecked ambition.
