Help please. Urgent advise needed for work

Okay so I really need some advice and this might be alot to read. So I work at a referral vets as a theatre animal care assistant, have been working there for nearly 2 years now.

But I think it was around February last year I was put on a performance plan for some small stupid things like not intialling jobs I've completed, but some that I do understand as I may have not been so focused at that time.

So we had constant meetings with my manager some times there were improvements and then they seemed to find something.

As I admit one of my weaknesses is induction where we have to clip the patients for surgery, and I'm not good at remembering the clips due to my dyslexia and also trying my hardest to avoid clipper rash and accidently clipping a bit too much.

So can't remember when it was but they gave me 4 weeks to improve and then we had a meeting to discuss if it was going to go to HR, which it did since there was an incident where after surgery a French bulldog was regurgitating and I wasn't fast on my approach in assisting.

So yea it went to HR and had a hearing, I've tried to follow everything they've said but I feel stressed due to the situation and doubting my self.

Unfortunately I've landed up getting a verbal and written warning, I've got a hearing next week to see if I'll get a 2nd written, and I felt really confident that I've done everything they've said and they couldn't bring anything up like I made sure to use close loop communication and being proactive, checking the clipper temperatures and not clipping too much this time.

I spoke to one of my managers the week before and she said she hasn't received any bad feedback from anyone, and also said that since that the next week I'll be in induction clipping that I can look at the clipping bible to refresh my memory or asking Alisha and Chloe (my boss) if needed for guidance.

So I literally hate induction so much, but I tried my hardest and I admit I am slow at clipping and I had one patient that did get some clipper rash but it had really sensitive skin so it was kind of unavoidable.
Also, didn't help that Chloe knows that I'm not confident in that area and left at most points for meetings so I was on my own.

I then asked Chloe to have a meeting last week to see if she had any feedback so I would know what would come up next week, she first started saying I don't have any feedback giving me hope and then started saying all this:

1. So, a couple of weeks ago I worked a weekend shift with a new assisant to theatre (used to be relational, so still worked in theatre but now works full time in theatre) and I worked with her talking her though the surgical kits since we had a huge amount of kit that day.
And chloe said "an asssiant from the weekend felt you didn't know what you needed to do with the kits." And it's like she clearly means Lily who I worked with which is cheek as she doesn't know any of the kits I literally had to go through all of it with her and did all of myself.
But I'm like is Lily being a two faced *****? As lily literally said to me and claire that I was a huge help and all that.

2. On one of my shifts in induction, I had to clip a dog for a sternotomy clip. And it should be noted that I'm hardly in induction maybe like once a month ans I'm also part time, I checked the request form and couldn't understand the landmarks requested, it wasn't in the clipping bible so I asked chloe.

And then she said in meeting that I should know the clip by now, like I said to her I'm hardly in induction then she's like yea well people go through induction and they know the basic clips.
I don't see the problem with me asking to be honest, claire even said don't be afraid to ask for guidance, just feel like that's stupid at least I didn't clip it wrong.

3. And then I clipped a dog on the neck which had sensitive skin, constantly was checking the temperature of clippers and changing them since the skin was turning pink and wasn't applying heavy pressure.

Surgeon came out to check the clip and said it was fine, but then pointed out where he was going to do his incision which had still quite a bit of visible hair which I thought I would try to clip, then one nurses came out and I showed her the area saying I don't think I can clip anymore off without causing more of a clipper rash then she asked if the surgeons have check it which I said yes and I finished up with preping the clip.

Chloe was like in my meeting, that I shouldn't of continued clipping after the surgeon checked it, and I said I was trying to get the visable hair off.
Cause she had previously failed me on clipper rash for my hearings, so she was like the dog had clipper rash maybe you should like of ways to avoid that.
Which I was like I tried my hardest to avoid it but the dog had sensitive skin, I changed clippers and checked the temperature, and she's like maybe you should of checked the blades maybe they were blunt.

Also, like to point that the nurse I was out in induction, pointed out herself that the patient had sensitive so it was unavoidable.

4. last one, so she brought out her phone and showed me a picture of kit list for screws saying that the intialling for the kit lists for the in and countersign column had the same handwriting but different intials. I said that day it was a busy day in sterilising so alot of us was helping, and katie was restocking the screws and left them of the table to be wrapped, I got the kit lists and wrote her intial to say she stocked it and mine as a countersign since I was wrapping it.

But she said well you should of signed her intials for her.
So I don't know I know I've made mistakes before in some of my meetings, but I definitely tried in this month but I feel like she's picking at little bits.

And I have a feeling, and normally if I have a feeling it's right that I will get a warnjng and I do it I won't take much for them to bring something up for me to get sacked which i don't want.

I'm thinking about potentially, asking to transfer departments for a different role taht way I would have a different manager, but I don't know if I would apply if they would take me on since due to my performance plan and hearings, as chloe can just talk yo the makers in the other departments and tell them and refuse me. Or I'm gunna have to leave, as much as I love my job I don't think I can stay without having the frear of being sacked and then not getting a reference.

There's jobs available in other departments but I don't know whether they will let me transfer with my performance plan and hearing despite the fact it will be with a different manager.
