Immigration Health Charge

Hi guys, I will be studying at Edinburgh University this year. I’m an EU-based student with EHIC, if I refund the NHS immigration fees, could I still work unpaid internships (as this is part of my later course requirement)? I have health insurance that covers UK costs, as such it seems unnecessary to spend multiple thousands of pounds on the NHS surcharge.
Any help would be greatly appreciated!
