EPQ questionnaire/survey

I'm doing an EPQ on the development of psychiatry and mental health perception in the Islamic world and am trying to see the differences between mental health perceptions of people who have been raised in different cultures/religions. I'd really appreciate if anyone could do my form, it won't take longer than 5 minutes and it's mostly multiple choice.

Hi everyone, I was wondering if I could get some help from anyone with my EPQ. I am trying to gather primary research using surveying and thought if it was possible for anyone in this chat to complete it.
