When you say, "The form for personal statement review no longer works", are you referring to this forum? Once there it will look empty (as you can only see your own threads in there) and there will be a message explaining this.
Did you click the "Start new discussion" button, entry your personal statement, and then click Submit? What happened? If there was an error message, what was that message?
(I'm just trying gather some information to be able to report the issue to TSR staff.)
