Medical School - oxbridge

So, I really want to apply for medicine, and have a few queries.
Firstly, is it worth applying to oxbridge and doing the extra research year? I am not sure whether it would be a waste of an application, and whether 6 years is unnecessary! And if you think i should apply, then to which one? How do i choose? If you have any questions that would help you answer this then let me know!
Secondly, except unis in london, which unis in England are good for medicine? I like the look of Liverpool and Bristol, and possibly York, and possibly oxbridge!
Lastly, which unis (and possibly courses) can be used as my back up choice? For example one with a lower entry requirement??
