Personal statement for sixth form

Heyy,I recently started year 11 and I’m currently looking at different sixth forms.My teachers have told me to write a personal statement for sixth forms /colleges.Could anyone please give me tips on how to write personal statement for sixth form as I don’t know where to start. I want to take biology,chem,maths and psychology.Thank you ☺️ These are some thing I’ve done so far to add on to my personal statement
• 1 week work experience at primary school
• Virtual work experience with NHS
• Cambridge online after school sessions
• Taster STEM lecture
• Form rep year 11
• Amazon web service competition
• Sports leadership
• Health and social care online work experience

Icl I did mine in a very rush so I would say add all work experiences and the qualifications
Add all of your achievements in terms of certificates and say why u want to go to that school
