Changing choices after application has been sent?

I have recently sent my UCAS application through school, however I am unsure about what Universities I have selected.

If i want to alter my choices even though I have sent my application, how would I do it?

If "recently" means that you're still within 14 days of the date on your "welcome" email from UCAS, then you should see a "Substitute choice" button next to your choices. You can use that if you change your mind. Beyond 14 days you can't substitute. See this page on the UCAS site for further details.
If its outside the 14 days, then all you can do is ask that Uni to switch your course choice at that Uni, you can't switch Unis.

The application is still with my college, and i cant see a welcome email anywhere in my inbox. Is it best to contact my school to see if they can change anything, or wait it out for the welcome email for when the application is sent?

Also, how many substitutions can i have?

If it has not been sent to UCAS by your college then ask 'the UCAS person' to send it back to you.
You can then alter as many of your choices as you like and resubmit it to your college.
Get onto this first thing in the morning so that you can stop them sending it!

Ah, I'd assumed it had actually reached UCAS. If it has not, then simply follow @McGinger's advice.

In answer to your second question, you can substitute each of your five choices once. (If you want to do that within 14 days of it hitting UCAS.)
