DLML (Medical Declaration Form)

Hello,
I have applied to study full-time undergraduate (distance learning) course and SFE sent me a Medical Declaration Form that has to be filled and signed, so I can get a Maintenance Loan too. I have a disability that prevents me from attending the course in person (severe mental problems). My last medical examination was done in Ireland (Dublin), where I come from. I would like to know, if my Irish Clinical Psychologist (qualified medical professional) can sign the SFE Medical Declaration Form and whether this would be accepted by the SFE.
