Random Latin poem I can't find anywhere

"Carmina viverent semper, Homere, tuis" - I translated a poem containing this line sometimes during my Latin GCSE (2 years ago and I don't study Latin anymore). Does anybody knows where it comes from/ who it's by?
I remember really liking this poem and others on the same document, and it said it was by some Roman poet (Horace or Catullus or someone??), but I can't find it anywhere on Google.
(Translation was something like "songs will overcome death and make one immortal. Your songs, Homere, will live forever")
