UCAS Reference details

Heya, sorting out my UCAS stuff, just wanna clarify some of the details for inputing into the fields in the reference section. My reference is my former tutor and lecturer from a previous school, however she has since left and moved to a different institution in Australia. I'm just curious, for the address and organisation section, if I list only the current school she teaches at, as well as its address or both the previous school where I was taught and the current one - and their addresses. Thanks!
