My PA missed a OAR form on epad

Could any one advise me as I thought my epad was completed. However, my AA has informed me my PA and I missed a form in OAR. This didnt flag up as being outstanding, also due to accidental error she ticked wrong box saying I have been on action plan, when I have not. Everything else signed with some positive feedback and comments. Now I cannot get in touch with her, as she not in work or seen emails and epad locks in 24 hours. Will I have to redo this whole placement due to this error? I've worked so hard and am heartbroken. University have said I will fail. We had this issue with midpoint where she forgot to submit a form.
