Distance Learnince Medical Declaration form

I'm studying a distance learning course, SFE has approved my tuition fee but for my maintenance loan they require a Medical Declaration form signed by my GP/ Doctor explaining my disabilty. I'm currently having trouble getting this form signed by my GP, it's been over a month or so and I haven't gotten anywhere.

I'm here asking if anyones else has filled out this form and if theres a specialist or private doctor I could pay to fill out this form. I can provide proof and details of my condition.

Has your GP mentioned why there's a delay? Just wondering if it's because of an issue/query with the form or simply because of their workload.

If you're unsure who can fill out the form, it might be worth contacting the DSA team so they can confirm what's acceptable.

is the medical declaration form a new process for student finance. I'm in my 3rd year at open university. studying at home for disability reasons. the first 2 years I've uploaded gp letters and this has been sufficient in securing my maintenance loan. this year the same letters have not been approved and been told I've have to fill in a medical declaration form which I've never done before.

Have you contacted your GP Surgery directly?

Yes, it changed this year.
I sent a letter from my Psychiatrist with my application, but they now require the medical declaration form.
My doctor was happy to fill that out. I'm in processing stage atm with SLC.

I have just been asked for this to be completed, the GP letter uploaded weeks ago isn't enough. My GP has told me it could take a month to get the form completed and charge me £60 (again). My course started 2 days ago and my finance hasn't been processed... I am panicking!

I have had my form filled by gp after minths of chasing this. It was sat in the practice managers office for a while. I wanted them to make a slight correction in the date when I had visited as they got this wrong. The doctor thats filled out my form with all his details, no longer works here. Does this mean I have to pay for this form to he completed again? I am panicking as they charged me £50 for this
