Would I qualify? Is it too late?

I finished sixth form some years ago, leaving with quite poor grades in Chemistry, Maths and Biology (CDD). When I left, I went into a degree apprenticeship and finished with a First class (Hons) in Chemistry and I have been working ever since.

My dream in sixth form was to go into Vet Med but my struggles with depression resulted in my terrible grades. I am now 26 years old, if I applied to Vet Med, would I stand a chance?
I know it is too late to apply for this year, but is there any chance of getting in through UCAS clearing or would I need to apply for an access course? (or is it also too late to apply for an access course?)

Get yourself on an Access course, complete the diploma with the necessary grades and you'll be good to go.

It’s never too late! I am a mature student who will be starting this September after completing an access course. I would check the entry requirements with the university as you would be able to apply for the 4 year programme as you already have a degree? Do you have any veterinary work experience/ animal husbandry experience as you will need this to apply.
