UCL Extend MPharm Fitness to Practise self declaration form

If invited to do the online test for mpharm on monday, you need to complete the fitness to practise self-declaration form, but after I made an account on UCL Extend I can't find it at all? it just says "UCL School of Pharmacy Application Course MPharm" and then "you cannot enrol yourself into this course" but there's nowhere where it says fitness to practise and the email I got about the test says it should be in that section?? Someone please help i'm begging you 😭

I thought it was okay like I wasn't 100% confident with my answer. regardless I finished with 9 minutes left. I thought that the math and chem questions were rlly easy. The other two sections were okay. I didn't research at all for the essay so I was a bit worried but I guess it was okay?
