Graduate Entry Medicine A101 funding timelines

Could someone kindly clarify:
i) when we should apply for SFE?
ii) whether we need to apply and how so for the NHS bursary?
iii) whether we need to apply and how so for the portion of fees covered by the NHS
iv) whether this is different in England vs Wales
