Student Finance wants more evidence?

I just saw got an email from Student Loans and it seems to want more evidence of income or something and says to download an Evidence Cover sheet. Where do I find this? I thought I completed everything.
I have already submitted evidence for DSA now it wants more evidence for extra Maintenance Loan when it didn't ask before. Where do I go for this? And where do I find the Evidence Cover sheet as I can't find any links for it.

Universal Credit and PIP

I've found the bit where you can send screenshots underneath where I sent the DSA evidence to. I uploaded 3 monthly payment statements, PIP evidence and also evidence of Council Tax Support. Hope that should be enough.
