At least for SWE there was a screening form that was sent out. If you haven't received it by now I'd lean towards you having being rejected. No harm in emailing them though to get a confirmation of your current status.

I got an email to fill out a form for data analyst, yes. Although it was the same form as the one I got for SWE.
