Postgraduate DSA, DSA1 form

I'm reapplying for DSA due to moving university for my MSc rather than doing a combined masters. I've seen I have to use a DSA1 form and have begun it, but there's a portion where someone from the university has to fill it out. I have an offer for next September, and obviously I want to get this sorted out before hand so the support is in place for when I start the course as I'm going to need slightly different things due to a different style of course and uni.
Who in the future university do I ask and how do I get in contact with them? Do I send them the whole form or just the relevant pages? Should I do so before I fill out the rest of it or after? This has got my banking and passport information on it, so it's not exactly something that I want to go sending at people willy nilly.
