How do i put these on my UCAS application as a ucas extra

An applicant can't enter these directly.

There is space in the reference for the referee to provide relevant details. Many unis also have a external form for applicants to submit mitigating circumstances - but this is typically used if they narrowly miss their conditions, rather than lowering the offer conditions being given out.
