Can I defer my insurance and apply to my first choice next year again?

Just that, really.

Do I have to reject my insurance to be able to apply again next year?

You can't hold any deferred place AND reapply next year.
On Results Day if your Firm accepts you, your Insurance disappears and you can't get it back.
If you dont go to Uni this year, you could reapply to any of this year's choices in next year's application.

Thank you. For example, if I have an offer from Uni B, but got rejected from Uni A (my first choice). I'd like to defer my place in Uni B, and re-sit my A levels to improve grades.

I'd like to decide what to do (reapply to Uni A or not) after I get the results. Does this sound like a feasible plan?

No. I'm planning for next year. I'll be applying this December.
The short answer is that you can't apply in the next cycle whilst holding a confirmed place, and UCAS won't let you carry a conditional offer over to the next year.

So in this scenario if you were accepted by either Firm or Insurance choice, you would have to be released from that place before applying again with achieved grades. But you can apply to the same choices again.
