Distance Learning

Hi,
I've applied for an open university that is online due to mental health reason ive completed all the documentation for student finance other than uploading supportive evidence, ive been asked "What can you give us to prove you're studying a distance learning course due to your disability, mental-health condition, long-term health condition or specific learning difficulty" and ive got no clue what to give them and no idea what an access center report is any help would be greatly appreciated
