All DSA 'paperwork' is done through the government's student finance website. You upload documents though there and everything you need to do is detailed there: https://www.gov.uk/disabled-students-allowance-dsa/how-to-claim

Is your funding body Student Finance England? If so, I think you can either scan your form and email a PDF of the completed form to [email protected]
or you can post to: DSA Team, Student Finance England PO Box 210 Darlington DL1 9HJ
