DLML Medical Form to prove necessity for distance learning on NHS

I need to get my NHS doctor to fill out this form. How do I do this? Also, should it be free or do they charge? Additional information, I am on Universal Credit with LCWRA. Also, I don't have access to a printer or scanner and I cannot use public transport or go to a library due to distress caused by my mental health condition in face-to-face settings (I live within walking distance of my small NHS clinic which is quiet. I find face-to-face appointments distressing but I can push through it because it's not crowded and is familiar and nearby). Do NHS print/scan legal documents like this?

Hi there. This is something you would need to discuss with your Doctor/GP as we wouldn't have access to this information.

Once you have the completed form from your GP/Doctor, you would upload it to your online account, it doesn't need to be printed off. Thanks, Leah.
