Completed IB Diploma - how to input grades

I am applying 2 years after I obtained my IB Diploma. For the education portion of UCAS, do I write IB as the qualification and input each subject as a module (save for the Extended Essay and TOK which is also allowed as an input for qualification)? Do I need to send my IB results via IBO for each university I am applying for or do I need to wait for the unis to request one?

Most UK Unis will request a copy of your transcript, but you could also share via IBO and tell then you have done this.
