I'm sorry. It's so nerve-wracking and even good drivers can have it all go horribly wrong on test day. My advice is just to keep practicing. Do as many mock-test scenarios as you possibly can with your instructor and work on those weak areas specifically.

Re the mirror fault - When you do the mirror check in 'mirror, signal, manouvre', make sure you visibly move your head to make it really clear to the test examiner that you are checking your mirrors. Sometimes, if you just flick your eyes to the mirror but keep your head still, it can look like you haven't checked mirrors. So do it in an exaggerated way, you might feel a bit daft, but you'll make it abundantly clear that you are actually checking.
