I guess it depends on what course you are applying for and what subjects the A* and B are respectively in.
Suppose you apply to a course requiring A*AA (but requiring at least an A in some subject) and then achieve A*AAB, where the B is in the aforementioned subject.
Even with the impressive set of grades, you won’t meet the offer requirements and may not be able to take a place on the course.

If you mean, in very basic terms, does a 4th A level mean you are more likely to get an offer - no.
