Unhelpful personal tutor

I had written my personal statement before the summer holidays and asked my personal tutor to go over it with me, which he agreed to do. However every time I have arranged him to do so, he has cancelled last minute or not turned up. I got fed up of waiting and have asked other teachers, and my older sister who gives personal statement advice at Oxford and everyone has been very happy with it, so I felt I was in a position to submit so I did (over a week ago). However my tutor has said he will not write my reference because he has not gone over my personal statement. The only times he has suggested to go over my statement, do not work for me as I have caring responsibilities. What am I meant to do? Also he's making me feel like it is my fault and that I've gone behind his back applying, I don't want a bad relationship with him and sure he means well but today is my schools deadline and he has done nothing to help.
