Any thoughts on 5th choice with Medicine application for other 4 choices?

I am deciding on 5th choice. Don't want to put in Dentistry, Pharmacy, Physiotherapy, optometry, paramedic or other health related as my 5th option.
I'm wondering if Engineering is a good 5th choice? Looking to do some shadowing in a local factory in December.
Anyone in similar situation?

Would you happily study engineering if you don't get into med, or would you take a gap year and reapply?

I think at this point, I am trying to pick a choice I would be happy to pursue. so, the shadowing might help me be more certain
