how likely is Cambridge to offer me a 4 alevel grade conditional offer

I applied to Cambridge medicine with 4 alevels (3x stem + latin) but I really want to drop latin as it looks like we're not even going to finish the course by study leave and I don't really have time to learn so much of it myself considering it's my 4th subject. I feel like I'm going to be rejected anyway so there probably won't be a problem with me dropping my 4th but if they did give me an offer how likely are they to make it 4 grades?

I have been here before with someone I tutored; got a 4 ALevel offer for English for Cambridge, asked the college if they could drop one, they said yes, happy days.
I would suggest waiting (only a week to go!), and then, if you do get an offer, ask. They won't withdraw you offer due to you asking, and you might get lucky. If not, at least you will be working for a reason!

do you think if they give me an offer it will 100% be 4 grades, or is there a chance they may just make me a 3 grade offer for my STEM subjects like most other unis do?

If you get a three grade offer you must still ask about dropping the 4th and tell your other unis and UCAS. Otherwise your results won't match up in August and you application could be considered fraudulent.
