Self-Declaration Form

Can someone kindly take me through how to properly fill out a self-declaration form? I did fill it before but I got a notification that I didn't fill it properly.

Are you referring to the terms and conditions page for the application or are you referring to a statement about yourself to send to your university?

It's for the MA Social Work at the University of Hull. It's called a Self-declaration form. It's before a DBS check. The form is in 4 parts

1. Introduction
2. Criminal Offences and related legal records
3. Disciplinary record, professional conduct, and notifiable listings
4. Overseas Police Check
