I haven't come across this.
I'm no expert but out of curiosity, I watched a documentary. A lot of porn actors rely on that. Simply because it's strenuous to "perform" so often. Everything is filmed from an angle so you can't possibly see.

well depends on the director and quality of the movie. If you watch some Peter North or Milf Hunter - it's not difficult to see that it's real.
