Yes, it seems most likely to me that (at least when done to men) it is supposed to insinuate that they are being cuckolded, traditionally represented by a pair of horns.
apparently, if to an italian person, you get the index fingers of each hand and wiggle them up and down, it indicates such a thing. no need to say they'd get very very offended by it.
Well there is definitely a story behind it. The act is more commonly known as doing "bunny ears". I'm not sure about the stag's horns story... but if someone can find a good link explaining how it all started.
