if you are using word use insert symbol or something, then select a square or whatever you are looking for.

Or if you want electronic tick boxes goto view|toolbars| then either Control Toolbox or Forms depending on which program you are using.
Tick boxes etc are all included on that toolbar, you'll have to assign a macro to them if you want the ticking to feed into something else (like a spreadsheet)
