    They will not voice any such opinion about you, or shouldnt anyway it would be unprofessional of them, if you give them a note that explains everything to them, i dont see a reason why it should be a problem. If i was a teacher i wouldnt even make a issue out of it.

    Dont be scared to talk to them, they are there to help and im sure will be very accomodating of everything.

    They aren't going to mind if you have a medical condition that prevents you from doing certain things in lesson. They most certainly won't judge you for it.

    Not at all, if anything they'll be pleased that you brought it to their attention as no teacher would want to force that situation upon someone.
