How to apply for DSA without a student loan?

Does anyone know how to do this? I go on the gov website and they say click here if you need a DSA but are not applying for student finance, and I click there, do the survey and click on the single main form I’m presented with and it has nothing about disabilities or support at all in the whole form. It’s like the normal student finance form. Does anyone know how to get the right form, or any DSA form? I’ve been trying for ages.
