DSA - which form?

I have already sent in my main loan/grant form (in July), and haven't had a reply. I ticked the box on it saying 'please send me a DSA form' but as they are not replying to me I want to send it in anyway. Which form do I send though? There seems to be a long and a short one on the website, and the long one has a section that has to be completed by the uni which isn't possible because I haven't moved yet.

You don't need to fill out section 5, it says:
If you are a full-time undergraduate who does not
study by distance learning go to Section 6

If you've done your loan form, it's the short one you need.

Thank-you Sabretooth, I totally missed that.

I've filled in the long one now ... I think they might not know who I am from the short one, as I haven't been sent a reference number or any acknowledgement of my main form at all.
