See section 3, "Appeals relating to access arrangements, reasonable adjustments and special consideration", of this document.

Note that the appeal must be made by "a head of centre (on behalf of a candidate or a group of candidates)" - i.e. your head - not yourself. The document also points out that before formally appealing, it "may help if the appellant discusses the situation with the relevant awarding body". Has your school been in contact with AQA at all to establish why special consideration was not granted? If not, I suggest your school contacts them.

As, "The appeal request should be made within 14 calendar days of receiving the original decision and should set out clearly and concisely the grounds for the appeal" it potentially too late for a formal appeal anyway.
